Loneliness and social isolation are widely recognised as harmful to the health of older adults, but frontline care workers have long lacked a simple and objective way to identify those at greatest risk.

Researchers at Hong Kong Polytechnic University (PolyU) have developed a set of community-based preventive approaches to address social frailty among older people, including a new screening tool and nature-based interventions at care centres.
